* in these clinical cases, diagnostic and therapeutic management is discussed from admission to discharge in 4 S’s: Start (empirical therapy, timeliness, cultures, rapid diagnostic tests, documentation) Safety (allergy, interactions, adapting to renal function) Streamline (re-evaluation, de-escalation, IV-oral switch, PK-PD alterations) and Stop (duration of therapy)
